Making an appointment:

Patients are seen by appointment. Same day appointments are available if needed. Please call in advance for your appointment. When leaving the office please be sure to schedule your follow-up appointment. We make every effort to stay on schedule, although emergencies do arise. If we do need to cancel the appointment and reschedule due to an emergency, we will do our best to contact our patients beforehand. Bring to your appointment:

· Updated Insurance cards
· Photo ID
· Updated medication list
· Please complete the new patient packet, and bring it with you to your appointment. If not, please arrive at least 15 minutes before your appointment
· Any imaging CDs, if the doctor needs to review


New Patients:

Please have your referring doctor fax us your referral before scheduling an appointment, 925-680-0212. You can print the new patient packet from here and bring it with you to your appointment, along with your updated insurance card, photo ID, updated medication list, and any imaging CD’s if applicable. Please be sure to arrive fifteen minutes early for your appointment, to allow time to review paperwork, insurance coverage and collection of any applicable copays.


Cancellation and Rescheduling:

We ask that you contact us at least 24 hours before your appointment, to cancel or reschedule. If not there will be a $50 charge for all missed appointments.

With five different offices, and affiliation with four different hospitals throughout the bay area, we are sure you will be able to find a location convenient for you.

Refill request:

Please allow at least 3 business days for refills. Please contact your pharmacy and have them fax us your refill request at, 925-680-0212. For all narcotic refills, please contact our office directly at 925-676-2600, once the request is completed by your physician, you will be notified and will need to come to one of our locations to pick up a written prescription.
